Blue and Yellow, Blue and Yellow...

Like the ever popular Wiz Khalifa song, blue and yellow seemed appropriate here.  I couldn't help but notice how I am unknowingly drawn to Pittsburgh and/or Pennsylvania travelers at work.  It's always a fun surprise to learn how close to home my new friends are.  I don't understand the attraction but I've met a lot of Pennsylvania people here.  Go Keystone State!

For those friends who are not from the area, we teach them about winters, hockey, zamboni machines and mullets.  I refer to the guy who cleans the floors on our unit Johnny Zamboni when he comes screaming through the unit and we can't hear ourselves think.  The basement also employs a zamboni-like machine to move trash and other items.  I've learned how few people on the west coast know what this is.  Yinz humor doesn't carry over here..  sad.  Someone asked "Is it a hot dog?"  "No but now I am hungry for a hot dog."  Very entertaining to hear what they THINK it is.

Unfortunately I won't be home for Christmas this year.  I'm not allowed to work Christmas because Kaiser pays 2.5 times pay and regular staff will murder to work this day.  On the bright side, I work no weekends this schedule!

The nice thing about traveling is that there are always people to hang out with and there are ALWAYS things to do because these travelers also have no holiday plans.  I am going to be busy these next few days.  Christmas day, I am going to Santa Monica to a friend's house who is hosting brunch.  After that, I will go home and cook a Paleo Christmas dinner and host my friend, another traveler.  We will also be celebrating our birthdays because his is near mine and he won't be in California anymore when it's here.  The day after Christmas, I was invited to help decorate the Kaiser float that will be in the Rose Bowl Parade.  I'm staying busy taking every opportunity presented to me here.  I have had a pretty busy and opportunity-filled year.  I just looked back on my "last 12 months" photos in my iPhoto application.  Wow.  Reflection.

Anyway, enough sappiness.  Let's get back to what is what in sunny California!

MELE KALIKIMAKA, N'AT!

I joked that this was going to be my Christmas card photo.  Then my match.com profile picture.  I don't even want to imagine the freaks that would attract!  This is actually what it's really like here:

Still no coat!
Though this is from a few months ago, not much has changed.  Just my skin color.  I am pale again.  I can recall exactly one day where it felt like I was home because it was windy, cold and leaves were blowing around.  I could also smell fireplaces burning.  Yum.

I did most of my in-store shopping at The Grove.  Partly because of the store selection but mostly because I needed to see decorations to get into the spirit.  They helped a little but the sun and blue skies distracted me.  Pretty...

Oh yeah,  I was pretty close to our old friend Albert Clifford Slater from Saved By The Bell.  He's really pretty up close!

I made sure to get my holiday nails on too.  I am pretty pleased with them!  Less pleased with how bad my phone camera sucks :(  Only a few more weeks til I get a brandy new one.  All of my pictures seem like a dream lately.  Partly because it feels like a dream being here maybe.

 It is December!

This past weekend, the monitor tech and my fast friend, Roya, invited me to the Kaiser CSU/ICSU Christmas Party.   Travelers don't go to this.  It ended up being kind of lame with like five people on the dance floor and the dinner was chewy and cold.  BUT!  In doing this, I think I built some bridges at work.  People have been nicer to me and not just dismissive like, "Oh she's just a traveler."  It was worth it if that means no seven to eight patient days, for sure!

These photos were shared with me, sorry they aren't foggy but dark instead.

Roya trying to teach me how to samba, oh brother.

My Date

Sunday I did something I've never done before.  I participated in a Sunday Funday event and watched almost an entire Steelers game (See, I met Pittsburgh friends!).  They were on the big screen!

I am always trying out new paleo recipes.  This one was a different chili than I had made before.  It was zucchini and peppers and meat.  Over mashed cauliflower.  Delish!

My parents sent me my Christmas package that is to not be opened until Christmas per strict orders.  My friends who used to live here (Bryan and Michelle) sent me  a package too with no instructions so I opened it!  Christmas and birthday wishes!  How very sweet of them to think of me!

Dear Santa, I REALLY need a new camera.  This is frustrating.

This is just a really sweet instagram that I saw this week.  I love it!

There are patient stories that I am leaving out and world events that I've skipped over.  I just wanted to get this posted so I could wish everyone a Merry Christmas.  Though I can't physically be present or buy presents for everyone, I wanted to shout out my love for you guys.  I miss yinz.  Also, Happy 2013!  The best is yet to come, right?

XO

Say, what's in my future?
  • Belly dancing classes (I'm serious)?
  • Cooking classes?
  • Crossfit (I know I keep threatening, but with all this paleo, why am I not doing it?  2013!!)?
  • DC in the spring?
  • Which if yes, this means another cross country trip is in my future too.  Pray for my VW.
  • Nature hikes before I leave?
  • A NEW PHONE.  Period.

If you hate it then you better put a moustache on it

So Movember is over (Kind of about moustaches, really about men's health, namely the prostate).

We supported it (see below).  Check out your man parts, men.  I know, we are nurses, but don't look at us, sickos.

Under the glasses I was hiding what my real expression is.  My homemade, 'born this way' face.  My siblings and I got a chuckle out of this e-card.  It's the truth, everyone thinks I am mad and unapproachable and I feel like Lady Gaga explaining all day "I'm not upset, I was born this way."

*****************************************

But the other day... The other day I was pretty upset.

I am on a Cardiac Surgical step down unit, which is not ICU where I was.  Patients have a lot of heart and vessel surgery and placement of pacemakers here.  Don't get me wrong, I am refreshing and relearning the heart well enough but I do feel like I am getting more dumb as a nurse, partly because as a traveler, you are given the unpopular and otherwise difficult patients.

A usual assignment here consists of 2-3 patients, usually with 1-2 discharges to home, opening you up for an admission or assuming care for another patient who is already present.  At most, you should see 3-4 patients in one day.  As of late, and I don't know whose moustache-y idea this was, I have been getting 6-8 patients in one day.  Not easy peasy, admit and begin care or end care and discharge.  These were some pretty tough ones with nurses in the family, complicated and exclusive family visitation (since when did I become your doorman/security too?),  and lots of unnecessary legal threats.

I think I already relayed the patient that I admitted for 4 hours and simultaneously while discharging her I was admitting her neighbor who arrived with crushing chest pains, only spoke Spanish, had an irregular and fast heart rate with blood pressure in the 250s.  Then I swapped this one out (missed out on a real ICU like bedside procedure) and assumed care for another mess of a patient.  And his family.  "Can he have a lemon packet?"  Really.

The other day I walked into work and should have known with the moon and the report I got from the night nurse that it was not going to be a good day.  He neglected to tell me important information about one difficult patient so I walked into a poorly cared for, dominating, young, very sick and unhappy man.  We had started from the top, immediately "I want to speak to a supervisor!"  Oh brother.

I had to take him off the unit for a procedure that was scheduled to last several hours.  Not once did he make eye contact and wasn't cooperative with any assessing of him.  I let it go and thought, he will be away, readjust his attitude and in the meantime, I will fix up his room and he will be excited to be back, hopefully feeling better than our first meet up.  In the meantime, my other patients kept me busy with a discharge, phone calls and transferring to another unit for a procedure at the bedside that could not be done on our floor.

Sparing some details for your boredom and his privacy, I picked him up early from the procedure because he had ended sooner than expected.  Still no eye contact.  C'mon guy, I can break almost anyone!  As I wheeled him backwards into his room he was already shouting to his wife, "Call my family!  Get them here to witness! Get my lawyer on the phone, I've been neglected!"  WHAT.  If anyone has ever worked with me (Erica), you can attest that I OVER care for my patients.  Erica (a nurse from home) once said, "If you had two brain dead patients, you would still somehow be running late on charting because you would be in the room with them."  I was so blindsided with this day.

The patient relayed to his wife that I had ignored him and neglected him (he was not on the unit long enough for me to neglect anything).  He said that I didn't answer phone calls from the nurse who was caring for him.  I was unable to because I was in another room with another patient and the charge nurse actually took the call and relayed the information to me.  So somehow, he took that as me neglecting him.  I know, he was just a mean and mad man, upset about the situation he was in and taking it out on his supposed submissive (me).  When I heard his take on the day I responded by outlining my morning, reinforcing policy and patient privacy and exactly what happened with his nurse's phone call.  His wife butted in and said "Please don't fight with my husband.  He is very sick."  WHAT.  WHAT IS HAPPENING!?

I shut my mouth and just focused on tasks at this point.  I collected vital sign information, reviewed the medications he was ordered and asked if he felt well enough to swallow pills.  Then I asked how he liked to take them, "ice water."  I told him and his wife that I would be back shortly with the medications and his water.  I stepped out of the room and immediately began shaking.

Then it happened.  I tried to send his blood work to the lab and one of my nurse friends asked me an unrelated question.  I looked up and just started bawling.  I backed my way into the copy room and my friend followed me.  I just unloaded everything about this patient's unfair treatment toward me while he was demanding that he "just be treated fairly."  Same here buddy, same here.  I am so mad at myself for crying at work.  So mad.  I can usually turn a bad situation good and work my butt off to bring the unhappy patient around.  Not today.  Now my eyes looked like they were bleeding.  The charge nurse met us in the copy room, I relayed what happened, she reinforced that it wasn't me but that he seemed to do better with male nurses.  She reinforced my worth to their unit and suggested making a switch.  She found a male nurse to take this patient from me.  Relief.

I took over his discharge, which ended up being a little more complicated than expected but I happily worked hard for this patient to get out the door.  At one point here I had zero patients and three waiting to get in.  I RAN to lunch.  Afterward, I got slammed back to back to back with report, admission, family request, phone call, procedure, doctor visiting at the bedside and repeat.  One guy I admitted was my favorite flavor, an 80-something guy.  I spent approximately three minutes in the room and went to catch my next patient coming into another room.  While I was admitting a different patient, my friend went into the room to help me pass some medications since they were not available when he arrived to his room.  The guy asked, "Where is that girl nurse?"  "She is admitting another patient, I am helping out.  Brought you some medicine."  "Oh.  Man, when I saw her, I thought I had died and gone to heaven!  She could make a small dog break a large chain!"  Thankfully this friend relayed that conversation to me.  Though I was drowning and having a rough day, that made it worth it.  I knew he had felt my care in the short time we spent together.  Another patient I had received came from a more intense unit so everything that was helpful was just removed from his body and the timer started now.  He needed a better IV, I had to make sure I watched the clock so that he peed, he needed a diet order and it was conveniently dinner time so he was hungry smelling everyone else's tray.  I have learned one thing from this unit, I will never be that ICU nurse that removed everything as the patient is heading out the door for a transfer, that's just mean.

This patient's wife was at the bedside when I arrived, the delivering nurse said that she pretty much was the nurse and would let you know what he needed.  She, however, needed a sleeper seat.  Again, am I taking care of you or your loved one?  Let me do a few things first.  She asked me several times and I also reminded her that I didn't forget her request, I just had a few things higher on the list at this moment.  GOSH.  His other family member asked me my name again as he was walking over to me.  Was he trying to shake my hand?  Oh no, "NATALIE...  I want to give you THIS."  I looked at a piece of paper he handed me.  It was a list of six names of family members that were unwelcome to visit.  It said something to the effect of "Under no circumstances should these persons be allowed in or be given any information.  If this request is not honored, we will take the appropriate legal action."  Again!?  Twice in one day.  Ironically, it was the same diagnosis and procedure.  Maybe I should start a study...

I thankfully finished my day and gave report to three understanding souls.  I prefaced report with the following and I am sorry if it is too much information but it is where I was that day at that time:

"I am sorry to give you report on this person, whom I barely know.  He was my 8th admission and I had a heck of a day.  Since this morning,  I laughed, I cried, I almost peed in my pants, I haven't changed my tampon all day and am going into toxic shock any minute now.  I have boob sweat, back sweat, pit stains down to my waist, I smell like death and my legs are throbbing.  That being said, I will tell you what I DO know."

Travelers are awesome.  They don't care and are confident enough to figure it out and take control of a messy situation if need be.  They don't judge other nurses for a bad day or a bad job.   Echoing my last post, I am thankful to be associated with the traveler team.

I can do this.  April will be here before I know it.

*****************************************

Now onto December.

There is no snow here, still.  It's pretty great!  I have had my balcony doors opened for a few weeks and the coolest it gets in my apartment is 69 so far.  There was one whole week of rain, light and then heavy for a minute, then light again.  I'm sure you heard about a storm hitting California and the north got it pretty good, flooding and some other damages.  We just had slow drivers and cloudy days.  My other traveler friends and I get a good giggle imagining native Californians in say, Pittsburgh or Chicago weather.  Oh boy!  A few drops and every highway is completely jammed with slow moving cars.  It really hit me when I said at work, "It took almost twice as long to get to work today.  A few drops and everyone slowed down the entire way here!"  I heard a native respond, "As they should!  It is so slippery out there!"  Wow, really?  I have yet to see a pile of wet leaves or any threat of skidding on the road.  What a bunch of wimps!

So, this is happening now:

Much happier news.  My parents landed on December 1st in Hawaii.  My dad arranged for them to get leis as they got off the plane.  Cute!  Now take those sweaters off and go enjoy the weather!

Thaaaaaaaaaaaat's better.  And unreal.  Looks photo shopped.  It's like a second honeymoon and that makes me smile.

This morning, they safely messaged once they landed from their helicopter tour of the island.  I had no idea Annie Ba-nannie had it in her!  I am going to rename her Dare Devil Grammy.  She won't be stopped!

She went THAT high.  Over ALL of that water!!

I'm just saying.  Watch your mailboxes...

Speaking of mailboxes, I'd better start working on some sort of Christmas shopping and cheer.  Cards and kids' gifts and secret Santa lists are in the works.  Three weeks?  See, April will be here before you know it.

And, Santa, if you are listening...

I've been a pretty good girl this year.  I've got myself and my life into some semblance of order.  I have had a great year and never wanted less in my life (other than an occasional hug and visit home to see family and friends again).  My list has gotten short is what I am saying, but these items...  I have been lusting after and wouldn't be upset if they arrived this year.

The dress, not the girl.  A Diane Von Furstenberg dress from last summer (2011).  Still loving it!

Any or all of these Michael Kors watches.  I originally feel in love with the rose gold but recently moved over to the pure gold and then I saw this black face.  OOOH!  I need to document the time of my life in style, no?

Besides nursing school, this really is probably one of the best better things that I have done for myself.  I still miss everyone and the comfort of that "home" feeling, but I'm doing it!  It's an odd balance of money, sun and fun versus home, comfort and a different, familiar warmth.  The holidays here are going to be a struggle but 'yinz guys' better fire up your FaceTime n'at!  Show me what  you are wearing and what nonsense you are all getting into!

Cheers of the Christmas flavor!

XO

A Thankful Bloggerito

(The font size won't cooperate so where I had trouble, I went larger instead of smaller.  I can't figure it out and I am done trying.  Just know it's not your eyes, it is me.  Enjoy!)

Sunday

I had this bright idea on my way home.  This would be the first time I've ever flown out of LAX.  What if I see a celebrity?  Let me catch their eye!  I bought these funky pants that I have been lusting over and finally found a similar version for a decent price.  I felt pretty unique!

I made it through security, grabbed breakfast and found my gate.  I found an open seat where I could spread out a bit with my bags and food and drinks without being obnoxious.  Once I was settled, I looked up and not 20 feet from where I was sitting was a tweener girl sitting indian style on the floor.. wearing the exact same pants!  Do you know how big LAX is?!  C'MON!  #uniquefail

I arrived in Pittsburgh right around 9pm and saw my dad and Abbey waiting for me so I did what I do best, I cried.

I am so thankful to be home!!

Monday

My first morning home, my dad wanted to visit his relatives in the cemeteries and deliver wreaths for Christmas.  He invited me to join him.

My dad wouldn't have known HOW HUGE this was, but he will now once he reads this.

I have not been to my grandparents' grave since they were each buried.  Thirteen years!  I was very close with them and when they died, they were cremated so I never got to see them or say goodbye to them.  I am happy I never had to see them suffering and was able to spend so much time with them, until I was 19 years old.  I was at work and at school and this time was unable to get there in time.  It all happened so fast, within 3 months of each other (look at the dates on the stone, that's true love).  I have had a hard time with death ever since.   I am pretty embarrassing to have around.

It was not as difficult as I had anticipated, it was actually a good feeling to be there AND I was able to keep it together.  Go me!

I am thankful for 19 years of being able to know and remember my grandparents the way my dad does.

My favorite picture of them <3

In the evening, I made dinner and Abbey and the kids came over.  My heart was bursting!

Vivian has always been very interested in girly things.  When I would get ready while I still lived at home and was visiting my parents place, she would ask plenty of questions.   She has grown to love pink and purple and make up and all things that sparkle.  That night, I asked her if she wanted me to fix her hair and put some make up on her.  Of course, she couldn't get upstairs fast enough.  She was digging her makeover!

Look at her hair!  She reminds me of Hayley Dunphy (Modern Family) from this angle.  So pretty!

Speaking of the Dunphy family, if dreams came true...

My friend made this for me because it's all I was talking about.  I love nerdy, goofy Phil.  Where is he!?

I asked for an updated picture in anticipation of seeing little Will (named after my grandfather).  Look at that baby cleave!

such focus!
I am thankful for tiny hands and huge hearts.

Tuesday

Tuesday morning I got an updated picture of Max, obviously missing me so much....

I am thankful for 8 years with this furball!

Tuesday night, I reserved a dinner with one of my favorite Pittsburgh couples (shown below), the Shadle-Shaffers!   Luckily, our friend Rachel was able to sneak out for the night as my date!

Erica and Jesse "Pittsburgh Dad" Shadle-Shaffer

Cinnamon and sugar on the rim, genius idea to compliment this piece of heaven we shared.
While we caught up and waited for dinner time, we shared some Pumking.  Now if you know it, you know what a huge deal this is each fall.  If not, let me fill you in.   California does not have pumpkin beer.  Well, they DO but what they have is pretty weak.  I hunted for the above beer, high and low.  I tried to order it online since it was nowhere to be found.  I would have to pay $150+ just to have 12 bottles shipped here.  I was really considering it!  It is THAT good.   Erica saved me a bottle, FTW!

Our reservations were for a little European restaurant in Shadyside called Cafe Zinho.  It is a BYOB restaurant, our favorite!  Food and company were equal in greatness.

Afterward, per our usual, we ended up at Harris Grill.  It's a nice central place where people can meet up easily.  It was so nice to see so many familiar faces that night!

I am thankful to have spent almost three years learning from and getting to know these wonderfully smart neuro-nerds.

Four reasons to have a stroke.

Wednesday

Out of desperation, I picked up a Gobblerito on my way home.  It is one of my favorite fall treats in Pittsburgh.  It is a burrito filled with thanksgiving foods!  Turkey, stuffing, mashed potatoes...  cranberry on the side, gravy on top.  You've gotta try one (next year since they are seasonal and gone now).

I am thankful for Mad Mex's gourmet creativity.

Nom to the nom

The first part of my Wednesday night included a hair appointment.  Finally!  Someone who KNOWS what she is doing.  Surprisingly, I cannot find a decent stylist in LA.  How is that even possible?! Breanne is a hair genius and fixed me up, made everything better.  Wonderful job, as always!

I am thankful for great hair stylists.

I think the owner was shaking a bit when he took this picture, ya think?

The second part of my night was Friendsgiving at the Lazzara House.  David made his "anything but turkey" cioppino, complete with a piece of lobster tail cooked using orange olive oil.  Kelly made several desserts and I tried every one.  It was all so delicious!

I am thankful for men who can cook, women who can bake and relationships that work.

Kelly, David, Amy, Dave and I sat around the dining room table sharing this meal and catching up. Our friendship is one where no matter how long we have been apart, we are quick to catch up and continue as if it's been just a few days.

After dinner and coffee, we got into our jammies and had girl talk before our adult sleepover and a 5k in the morning.

I am thankful for great friendships, adult sleepovers and giggling girl time.

Thursday Thanksgiving Day!

In the morning, Kelly, David and I got up and dressed for the 5k/Turkey Trot.  I have been doing this for a few years and invited them to come along.  Rachel also runs this race and reminded me to register before I got home.  It's really just rationalizing the amount of food I will be taking in over the rest of the day.

Found these gems in the backseat - 'Merica!

We made great time, found easy parking and arrived in time to not freeze or be late for the start.

READY...

...SET...

GO!!!

It was a personal best for me (I ran alone) coming in just under 30 minutes and not stopping once (impressive after a night of adult beverages and stuffing my face with food). I can't wait to do it again next year!

I am thankful for traditions.

And finally, Thanksgiving dinner time!

Will arrived with his parents and we hugged for most of the night.  Well at least that's how I tried to have it happen :)

Regardless of how it really happened, I got some sweet pictures that will last me for a little while until we hang out again.

Ohai, baby!

30 REASONS I AM THANKFUL TO BE HOME!!  XO

...and Uncle Gregg, if you are reading this, I am thinking DC for my next assignment!

Friday

I had to leave bright and early Friday morning for fear that the Black Friday shoppers would jam up the parkway.   Fooled again!  Everyone must have been waiting for the made up Cyber Monday because I got to the airport in no time, arrived at 6:50am.  Was in, through security and on the shuttle to the terminals by 7:03am!  Amazing!

While I was patiently waiting for my plane to board, I found another Paleo cookbook that was on special for Black Friday.  $30 off!  I haven't made much from it but what I have made and seen, pretty delicious looking and tasting!  Check it out:

...and beyond

I made dinner for my friends when I got back to catch up.  Paleo isn't for everyone as I learned when my friend Nick, from Tennessee, tried spaghetti squash.  Entertaining to say the least!

almost paleo minus the cheese

Max helped (or I tricked him with a Beggin Strip)!

"Spaghetti" with a roasted red pepper meat sauce

With Nick and Emily

Emily is also from Pittsburgh.  We were trying to do 412 but were a bit backwards on who was which number.  She may travel to DC too!  Judging from the picture above, this could mean trouble, muahaha!  Sweet, sweet trouble.

Back to the grind on my new contract and I already have some good stories to share.  Until the next post, enjoy!
